Answer Yes where the statement best describes your organisation today. All questions are negatively framed — a Yes means a maturity gap; a No means the described weakness is not present. The result is directional rather than a formal audit.
This is a directional diagnostic, not a formal audit. Scoring: questions 2–16 identify maturity gaps; 6–15 Yes answers suggest contacting Equidata. Question 1 is a leadership prerequisite — a Yes response overrides the score, because effective IBP is unlikely to succeed without active sponsorship by the most senior leader.
